For screening, I require at least ONE of the following:
- A photo of you holding your ID
- References of two providers you have seen in the past year, with your full name and a current photo
- Photo of your ID and LinkedIn
Please send your choice of screening via my secure contact form or email. Your privacy is extremely important to me, and all of your personal information is deleted after the screening process is complete.
Incomplete inquiries will not be responded to.
